Description
The Workers Health & Safety Centre (WHSC) Ontario offers a systematic form for reporting workplace incidents, such as chemical exposures, near misses, and safety breaches, in this printable Health and Safety Incident Report Form. In order to promote compliance with WHMIS 2015 and Ontario health and safety requirements, it assists employees and supervisors in documenting important facts such the incident's nature, the individuals involved, the contributing circumstances, and the corrective steps done.
