Case Study<\/strong><\/span><\/p>\n\n
Thomas is a 14 month old boy diagnosed with Trisomy 18. He is developmentally delayed and cannot sit up without assistance. He suffers from hearing loss, multiple heart defects, and seizures. Thomas requires 24 hour care and his health is fragile. He is fed 4 times a day via a gastrostomy tube, takes multiple medications throughout the day, and requires constant supervision as he can have up to 6 seizures a day. Thomas produces a lot of secretions and needs frequent suctionning. He has issues with reflux and his parents adjust his feed type and amount often. What is pediatric palliative care?<\/strong><\/span><\/p>\n\n
P<\/span><\/span>ediatric palliative care (PPC)<\/span> is defined by the World Health Organization as a special field of care that focuses on relieving the suffering of a child diagnosed with a life-threatening illness. This suffering can be physical, psychological, social, or spiritual in nature, and includes the suffering of the child's family members <\/span>(World Health Organization, 2018)<\/span>. <\/span><\/p>\n\n

What is End-of-Life?<\/strong><\/p>\n\n
End-of-life<\/span> is defined as the days or weeks leading up to, and shortly after, a person's death (National Coalition for Hospice and Palliative Care, 2018). <\/span>We must respect the child and their family as well as their wishes to preserve their dignity<\/em> and give them the best possible death.<\/p>\n\n

The death of a child is similar to that of an adult. No two people are the same and as such, no two deaths are the same. Some things to look out for include, but are not limited, to:<\/p>\n\n

Neurological changes<\/em><\/strong> - decreased wakefulness, decreased sensitivity to touch or stimulus, increased pain, increased seizure activity<\/li>\n\t
Cardiac changes<\/em><\/strong> - tachycardia, bradycardia, decreased circulation, pale or mottled skin color<\/li>\n\t
Respiratory changes<\/em><\/strong> - dyspnea, tachypnea, increased exertion, increased secretions<\/li>\n\t
Gastro-urinary changes<\/em><\/strong> - decrease appetite, increased nausea, vomiting, decreased food tolerance, decreased urine production, constipation, diarrhea<\/li>\n\t
Musculoskeletal changes<\/em><\/strong> - increased\/decreased muscular tone, spasms, incontinence<\/li>\n<\/ul>\n\n

Grief and Bereavement<\/strong><\/p>\n\n
Grief <\/span>is defined as the feelings and emotions experienced by a person in response to death (CHPCA, 2006).<\/span><\/p>\n\n
Grief manifests itself differently in each person. Some emotions experienced include sadness, anger, guilt, and helplessness. Grief can begin as early as diagnosis, not just reserved for time of death.<\/p>\n\n
Bereavement<\/span> describes the journey a person goes through after suffering a loss through death (CPHCA, 2006).<\/span> <\/span><\/p>\n\n
Support for the child, parents and siblings begins at the time of diagnosis and continues after the death of the child. <\/span><\/p>\n\n
Be open about your feelings. It's okay to feel sad and cry! RNs are humans too and we go through our own bereavement when a patient dies.
